Q. Where do I find my member ID number?
A. The member ID number is located in the upper right-hand corner of your member ID card. All member numbers contain the prefix HPL.

Q. How do I request a new member ID card?
A. You can request a new member ID card through the Member Portal. To access the portal, click on the MEMBER PORTAL link in the top right corner of this site. Once you are logged into the portal, click on: Card Request. You can also call Customer Service at 513-554-1100 or toll free at 800-367-9466 to request a new card.

Q. How can I check if a dentist is participating in the network?
A. Click on Find a Dentist, select your product (found on your member ID card), select your search criteria and then click search.

Q. How can I access benefit information?
A. Benefit information can be accessed via the Member Portal. To access the portal, click on the MEMBER PORTAL link in the top right corner of this site, log into the portal and your benefits are displayed on the member home page.

Q. How can I check the status of a claim?
A. Claim statuses can be checked inside the Member Portal. To access the portal, click on the MEMBER PORTAL link in the top right corner of this site. Then log into the portal where you can view finalized claims for you and your family by clicking on the claim search tab.

Q. How do I change my address?
A. To change your address through the Member Portal, click on the MEMBER PORTAL link in the top right corner of the site, log into the portal and click change address. Or you can call Billing and Enrollment at 513-554-1100 or toll free at 800-367-9466 to change your address.
